Big Update in Practical Endodontics 2024 Arnaldo Castellucci, MD, DDS In recent years in Endodontics there has been a true explosion of new technologies, new instruments and new materials, which made predictable many procedures that before were considered impossible or just made by chance. One of the most important revolution has been the introduction of the surgical operating microscope. With the use of the operating microscope everithing is changed both in non-surgical and in surgical endodontics. In non-surgical endodontics, every challenge existing in the straight portion of the root canal system, even if located in the most apical part, can be easily seen and then solved under the microscope, with magnification and coaxial illumination. In surgical endodontics, thanks to the microscope and to powerful ultrasonic units and tips, today it is possible to make class I cavity preparations perfectly along the axis of the root canal, threedimensionally cleaned and obturated, in a total respect of the root canal anatomy. As far as the new materials is concerned, a great revolution has been the introduction of Bioceramic Materials. The first one has been MTA, which can be considered the material of choice both in surgical endodontics (as a retrofilling material) and in nonsurgical endodontics (in direct pulp capping, to repair perforations, for the apical barriert technique in treatment of open apices). Recently, a new method to activate the irrigating soluntions in endodontics became available, eliminating completely the risk of extrusion, and a new generation of Apex locators is today giving us the possibility to be extremely precise in working length determibation. Finally, as it happened years ago for the use of the operating microscope, today the use of CBCT is spreading like wildfire,. The era of "radiographic interpretations" is over. Today we are able to "see the pathology", with the advantage of being able to carry out correct diagnosis and above all correct treatment plans. Thanks to all these revolutionary progresses, the long term success rate of root canal treatments is higher and endodontic therapy today is easier, more predictable and even more fun! |
Dr. Arnaldo Castellucci MD, DDS, Professor Micro-Surgical Endodontics School of Specialization in Oral Surgery World Top endodontic specialist, his clinical and teaching experience in microscopic endodontics spans almost 40 years. He has produced reference works and authored many articles and books of international renown, that helped train entire generations of dentists worldwide, and they still do. He is Past President of the Italian Endodontic Society S.I.E. and Active Member of the European Society of Endodontology E.S.E., where he was the Secretary in 1981-83. He is Active Member of the American Association of Endodontists A.A.E. since 1985. He has been the President of the International Federation of Endodontic Associations I.F.E.A. in 1990-92. He is now Assistant Professor of Endodontics at the University of Cagliari Dental School and Professor of Micro-Surgical Endodontics at the Specialty of Oral Surgery of the University Federico II of Naples. He has been the Editor of “The Italian Endodontic Journal” and of “The Endodontic Informer”. He is also the Founder and President of the “Warm Gutta-Percha Study Club”. Today is Editor in Chief of Endo Tribune and is Editorial Advisor of the journals Endodontic Practice and Endodontic Practice US and is in the Editorial Board of Clinical Dentistry. He is Founder and President of the Micro-Endodontic Training Center in Florence, where he teaches and gives hands-on courses on nonsurgical and surgical Endodontics. He published more then 60 articles on Endodontics in the most prestigious Endodontic Journals. |
